Catchment area

Before filling out a registration form, check you are in our catchment area using the map and postcode checker opposite.

The Practice registers patients in and around the surrounding area of Yatton Keynell.  

While the North and West edges of our boundary cover some South Gloucestershire areas, it may be in your best interests to register with a practice in South Gloucestershire as we do not have access to South Gloucestershire services.

Our South-East border runs along the A350 and we do not register new patients living on the Chippenham side. We also do not register patients living in Corsham, to our South.

Please be aware that if you move to an address outside of our catchment area you will be asked to register with another practice.

How to register

We are using a new online service called Register with a GP surgery that makes it easy for patients within our practice area to register with us.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number although the process can be quicker if you know your NHS number.

You can use the NHS website to find your NHS number.

Once completed, please allow approximately 3-5 working days for our team to register you. Please telephone our reception team if you have any immediate concerns.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one - visit our reception to collect them.

The Surgery

The surgery building is accessible for wheelchair users. If you require assistance getting into the building, please contact reception before your appointment time and we will endeavour to meet and assist you.

A wheelchair is available for use whilst on the premises, again please contact reception in advance of your appointment so that we can be ready to help you with this.

A hearing loop is available, please let reception staff know if you wish to use this facility.

 

Temporarily in our area?

You can register as a 'temporary resident' with the surgery if you are staying at an address in our catchment area for up to 3 months and require an appointment. If you will be in the area for longer than 3 months, please register as a permanent patient (details above).

To register as a temporary patient, please visit Reception to complete a temporary services form (GMS3).

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To request proxy access, please download the Proxy Access Request Form or collect a copy from reception.

Please note, ID will need to be checked when returning the completed request form. This is so we can confirm who you are and check it’s appropriate for you to act for the other person.

Requests for access can take up to 14 days to process.

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.
