New Patient Registration

The practice welcome new patients who live within our practice area. The Practice registers patients in and around the surrounding area of Yatton Keynell. Generally speaking, we do not register new patients living in or moving into Chippenham or Corsham. Please ask reception to check if you live within our practice area.

As it can sometimes take time for records to be forwarded from your former practice, new patients will be asked to complete a health questionnaire along with their registration form.

Medical treatment is available from the date of registration.  Please contact reception for further information.

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.

The surgery building is accessible for wheelchair users. If you require assistance getting into the building, please contact Reception before your appointment time and we will endeavour to meet and assist you.

A wheelchair is available for use whilst on the premises, again please contact Reception in advance of your appointment so that we can be ready to help you with this.

A Hearing Loop is available, please let Reception staff know if you wish to use this facility.

Please complete the following form and questionnaire to register with the practice (these are also available at Reception)

Patient Registration Form - please return this registration form to the practice and complete a health questionnaire to register. It would help the registration process if you could provide some photographic identification and proof of address when registering.

New Patient Registration Questionnaire - please download a copy of this health questionnaire, complete it and bring it along with your registration form when you register with the practice.

The registration process is very quick and, in most cases, your electronic medical records will arrive with us from your previous GP practice within minutes of registering.
