New Patient Registration

The practice welcome new patients who live within our practice area. See our boundary map to check if you live within our practice area or ask Reception.

As it often takes time for records to be forwarded from your former practice, all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with our practice nurse. 

Medical treatment is available from the date of registration.  Please contact reception for further information.

The surgery building is accessible for wheelchair users. If you require assistance getting into the building, please contact Reception before your appointment time and we will endeavour to meet and assist you.

A wheelchair is available for use whilst on the premises, again please contact Reception in advance of your appointment so that we can be ready to help you with this.

A Hearing Loop is available, please let Reception staff know if you wish to use this facility.

Patient Registration Form - please return this registration form to the practice and complete a health questionnaire to register. It would help the registration process if you could provide some photographic identification and proof of address when registering.

New Patient Health Questionnaire - please download a copy of this health questionnaire, complete it and bring it along with your registration form when you register with the practice.

 

The registration process is very quick and, in most cases, your electronic medical records will arrive with us from your previous GP practice within minutes of registering.
