New Patient Registration

The doctors welcome new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with our practice nurse.

Medical treatment is available from the date of registration.  Please contact reception for further information.

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.

The surgery building is accessible for wheelchair users. If you require assistance getting into the building, please contact Reception before your appointment time and we will endeavour to meet and assist you.

A wheelchair is available for use whilst on the premises, again please contact Reception in advance of your appointment so that we can be ready to help you with this.

A Hearing Loop is available, please let Reception staff know if you wish to use this facility.

Patient Registration Form - (also available from Reception) please return this registration form to the practice and complete a health questionnaire to register. It would help the registration process if you could provide some photographic identification and proof of address when registering.
